Job Title - RN Case Manager Schedule - Monday through Friday (8am to 5pm) / Hybrid (Mileage would be reimbursed) Location - Sacramento, CA Job Duties - + Coordinate care across multidisciplinary teams to ensure quality, cost-effective outcomes. + Manage complex cases from assessment through discharge planning. + Serve as a mentor and subject matter expert for fellow nurses. + Work both telephonically and on-site to support members' health journeys. + Collaborate with providers, families, and community resources to deliver patient-centered care. Pay - $50-55.29/hr What We're Looking For: + 4+ years of acute care and complex case management experience. + Strong knowledge of managed care and Medicare. + Excellent communication, critical thinking, and leadership skills. + Active CA RN license (Case Management Certification preferred). Pay and Benefits The pay range for this position is $50.00 - $55.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
