🩺 Requirements (Non-Negotiable)

• RN Case Manager with 2+ years Acute Care Case Management
• Strong discharge planning background
• Minimum 6 months experience at a Level 1 Trauma Center or large academic medical center
• Experience across multiple inpatient units
• EPIC Charting – REQUIRED
• 5x8-hour shifts – non-negotiable
• BLS required
• CCM preferred , not required
• RN, BSN preferred (ADN considered)

RN Case Manager – Discharge Planning

UMMC | Inpatient | M–F Days

💉 Compliance Requirements

• Flu Vaccine REQUIRED
  • Only medical exemption allowed
  • Must include provider documentation of past severe reaction
• Fingerprinting
  • Remote: allow 10 days
  • In-person: available at Jackson or Grenada campus the week prior to start

🗂 Role Overview

• Department: General Case Management / Discharge Planning
• Focus: Discharge planning (not UR)
• Caseload: ~15–20 patients/day
• Patient Population:
  • Primarily medical discharges
  • Some complex and social discharges
• Coverage: All inpatient units
• Hospital Type:
  • Only Level 1 Trauma Center in the state
  • High-acuity, complex patients, limited placement options

💻 Systems & Tools

• EMR: EPIC
• Referrals: naviHealth
• (Planning to transition fully to EPIC Care Management within a year)
• InterQual/Milliman: Not required
  • Must understand general discharge readiness

🧑‍🤝‍🧑 Team Structure

• Current CM Team: 15 CMs
  • Goal: 21 total (18 General + 3 Complex)
• Social Work:
  • 15 full-time
  • 17 part-time
• Reporting Structure:
  • Reports to Social Work Manager
  • Oversight by Chief of Care Coordination
• Support:
  • Will never work alone
  • RT & Pharmacy consulted as needed

🕒 Schedule

• Monday–Friday | 8:00 AM – 4:30 PM
• Weekends: None
• Holidays: Rotated as needed

👕 Dress Code

• Any color scrubs or business professional attire

🧭 Orientation

• 2-week department-specific orientation
• Includes general facility onboarding
